How Scope Medical collects, uses, discloses, and protects your information.

Our commitment

Scope Medical handles personal health information in accordance with Ontario’s Personal Health Information Protection Act, 2004 (PHIPA). The physicians who see you are the health information custodians for your record. Clinic staff act on their behalf and are bound by confidentiality.

What we collect

  • Identifying details such as your name, date of birth, address, phone number, and health card number
  • The referral from your family physician or nurse practitioner and the records sent with it
  • Information gathered during your consultation, including history, examination findings, test results, and the plan of care

How we use it

  • To provide your care and communicate with the provider who referred you
  • To arrange appointments and contact you about your visit
  • To order and receive laboratory and imaging results
  • To bill the Ontario Health Insurance Plan
  • To meet legal and professional obligations, including record-keeping requirements of the College of Physicians and Surgeons of Ontario

When we share it

Your information is shared with other health care providers involved in your care, including your referring provider, laboratories, and imaging facilities, unless you tell us otherwise. We may also disclose information where the law requires or permits it. We do not sell personal health information.

How we protect it

Records are kept in a secure electronic medical record. Access is limited to staff who need it to do their work, and staff sign confidentiality agreements. Please do not send personal health information by regular email; use the phone or fax listed on the contact page.

Your rights

  • You may ask to see your record or request a copy.
  • You may ask us to correct information you believe is inaccurate or incomplete.
  • You may limit how your information is shared by placing a restriction on your record. We will explain what that means for your care.
  • You may withdraw consent for future uses and disclosures, subject to legal limits.
