Your visit
Step by step.

Referral
Your family physician or nurse practitioner sends a written referral. The clinic books from that referral.

Scheduling
The clinic contacts you with the appointment date and time.

Before your visit
Gather your health card and a current list of medications. Arrive a few minutes early.

The appointment
You are assessed by an internal medicine specialist. A report is sent to your referring provider.
The visit
What happens at the clinic.
Your consultation is with an internal medicine specialist. The visit addresses the question in your referral and may include a review of your history, an examination, and a discussion of any investigations or next steps.
After the visit, a consultation report is sent to your family physician or nurse practitioner, who continues to coordinate your ongoing care.
What to bring
- Your Ontario health card
- A current list of medications, including doses
- Any recent test results or reports you have
- Reading glasses or hearing aids, if you use them
- A family member or friend, if you wish

Do I need a referral?
Yes. Appointments are booked from a referral sent by your family physician or nurse practitioner.
How will I hear about my appointment?
Reception contacts you by phone once the referral has been received and reviewed.
Is there a cost for the visit?
Consultations are covered by OHIP for patients with a valid Ontario health card.
What if I need to change or cancel?
Call reception at least 48 hours before your appointment so the time can be offered to another patient. Late cancellations and missed appointments are charged a $250 fee. See the cancellation policy.
Will my family doctor receive a report?
Yes. A consultation report is sent to the referring physician or nurse practitioner after your visit.
Emergencies
For chest pain, severe shortness of breath, or any emergency, call 911 or go to the nearest emergency department.
